Corey Jago is the Vice President of Home Health for Homecare Dimensions within the WellMed Medical Management Network. He earned his nursing undergraduate degree from Jacksonville University and a Master’s degree from University of Miami. As a Navy Nurse Corps Officer, he completed clinical and leadership assignments in the Intensive Care Unit, Emergency Department, Family Practice Clinic, and a Medical Training Command. In 2018, he retired from the Navy after 23 years. After retirement, he accepted the offer to become a VP at Homecare Dimensions. He currently oversees the home health operations for the six branch markets in Texas.

The genesis of our Hospital to Home (H2H) Program began at the peak of the COVID-19 pandemic in Texas, due to critical hospital bed shortages. Our teams implemented the project to ensure a timely and safe discharge of our patients from the hospital/SNF/ER settings to the patient’s home. The H2H Program expedited patient throughput and increased hospital bed availability in three specific market locations with assigned Hospitalists.
