Since the advent of the HITECH Act (2009), there has been exponential growth of the electronic health record (EHR). While advances in clinical decision support and limited interoperability have been made, limitations in EHR functionality still exist for clinicians. This is particularly evident for healthcare teams. EHR platforms were designed for individual end-users and lack functionality for active teaming in the clinical arena. Better EHR technology infrastructure is needed to optimize team-based care to optimally impact patient outcomes. The purpose of this Lightning Talk is to describe (1) primary care provider recommendations for using the EHR to support team-based care and (2) examples of tools available in primary care EHRs that may be leveraged to document and evaluate interprofessional teamwork. Information shared in this Lightning Talk was developed through a rapid cycle literature review, participation in webinars, focus group interviews with primary care teams, and discussions with local and national EHR experts. We triangulated data from each of these sources using qualitative thematic analysis. Primary care providers want improved functionality to work in teams and receive real-time feedback on patient care outcomes. Several of these needs and preferences may be met using patient dashboards that can be viewed by the entire primary care team. Direct communication portals in the EHR can be used to enhance communication between team members, including the patient and families, transitions in care, well-being for care providers and virtual care visits. Examples of these strategies will be included in the presentation. While it is a common belief that EHRs have limited capacity to support teamwork, the results of our literature review and interviews suggest that they may have greater capacity than we are currently using. We propose that EHRs have considerable promise to contribute to documentation of interprofessional teamwork and to demonstrating its impact.
